What is a Patient Centered Medical Home?

First of all, it is PATIENT-centered. With each encounter, we strive to place our patients first. Our goal is to address patient and family concerns at every visit. We will also discuss each child/teen’s health directly with him/her in an age appropriate manner. We will develop health goals in collaboration with our patients and families.

As your child’s medical home, Pediatric Partners of NKY will provide your child with expanded, better coordinated care. Our providers will work both with you and other health care providers such as specialists and therapists as a team to give your child the best care possible. You will have access to our providers through phone and over the internet via NextMD our electronic patient portal.

As your child’s primary care providers, we will:

  •  Learn about your child, family, life situations, and health goals and preferences. We will consider these and your health history every time you seek care and suggest treatments that make sense for you and your child
  • Take care of any short-term illness, long-term chronic disease, and your child’s all-around well-being.
  • Keep your child up-to-date on all vaccines and preventive screening tests.
  • Connect you with other members of your child’s care team including specialists, therapists, and behavioral health professionals and coordinate your child’s care with them as his/her health needs change.
  • Be available to you after hours for your child’s urgent medical needs
  • Notify you of test results in a timely manner.
  • Communicate clearly with you so that you understand your child’s health conditions and all your options.
  • Listen to your questions and feelings. We will respond promptly to you in a way you understand.
  • Help you make the best decisions for your child's care.
  • Give you information about classes, support groups, or other services that can help you learn more about your child’s condition and help him/her stay healthy.

We trust you, as our patient families to:

  • Know that you are a full partner with us in your child’s care.
  • Come to each visit with any updates on medications, dietary supplements or home remedies your child is using, and questions you may have.
  • Let us know when you see other health care providers including visits to urgent cares so we can help coordinate the best care for you.
  • Keep scheduled appointments or call to reschedule or cancel as early as possible.
  • Understand your child’s health condition: ask questions about your child's care and tell us when you don’t understand something.
  • Learn about your child’s health conditions and what you can do to keep them as healthy as possible.
  • Follow the plan that we have agreed is best for your child’s health.
  • Give your child medications as prescribed.
  • Contact us if you do not receive your child’s test results within the time your provider told you they would be available.
  • Whenever possible, contact us before going to the emergency room, so that someone who knows your child’s medical history can help manage your child’s care.
  • Agree that all health care providers in our care team will receive all information related to your child’s health care.
  • Learn about your health insurance coverage and contact your insurance company if you have any questions about your benefits. Our staff is always willing to help when they can.
  • Pay your share of any fees.
  • Give us feedback to help us improve our care for your child.
