Transferring To Our Practice

When you choose Pediatric Partners of Northern Kentucky to provide care for your child, we ask that you have your child's prior medical records sent to our office.  Our  practice functions most effectively as a Medical Home when patients provide us with complete medical histories and information about care obtained outside our office. 

For new patients, we need to have prior medical records in our office before we schedule your child’s appointment. To assist you in obtaining your records, a Medical Release form is available here , you can print it and send it to your previous provider or bring it to our office and we will send it to your previous provider for you.

Once records are received in our office, we can schedule an appointment to establish care.  At your first visit, our physicians will examine your child and discuss any medical concerns that you may have regarding their health and development.  We look forward to meeting you and your child!
